-Submitting a request for any package: proof of diagnosis is required. Forms of acceptance can be medicine bottles (with name visible), diagnostic papers, treatment papers, or an email from your doctor confirming treatment.

-Recipient must have a life threatening or debilitating disease, condition, disability, or special need. 

​

-Within the last month, if you have received a package OR item from another organization similar to ours, we kindly ask that you please do not submit request as it takes away from others waiting.

After reviewing the above, you may submit a request under the programs tab once you have deemed which category you fall under.

Question: How do I know if I qualify for a package?
